You are responsible for reading the Guidelines and understanding the requirements of holding a third party event for the benefit of UAMS. We appreciate your compliance and cooperation. Please ask questions about any Guidelines you do not understand. Submit the completed application to request approval to conduct a fundraising event for the benefit of UAMS. This application has been developed to make this process as efficient as possible for you and to provide consistency with UAMS policies and procedures.
